Enrollment in CereShift’s Monthly Maintenance Membership (hereinafter referred to as “Membership”) signifies your agreement to our terms and conditions, as follows:
This membership entitles the bearer to a designated number of IASIS therapy sessions per month. Sessions must be used within one billing cycle, or they will be forfeited. Membership sessions are non-transferrable and non-refundable. There’s no setup fee, and clients can cancel any time they like.
CereShift clients who have purchased at least 40 sessions, and who may benefit from regular maintenance sessions based on lead therapist recommendations, are eligible for this package.
Investment: You agree that you are financially willing and able to invest in this Membership by choice, and that by so doing, you are not incurring any economic hardship in any way.
Payment Authorization and Receipt: You give us permission to automatically charge your credit card or debit card as payment for your Membership without any additional authorization, and you will receive an electronic receipt.
Missed Payment: If there is a problem with the payment transaction or method, you will be notified by e-mail and will have a 5-day grace period to make the payment. Membership sessions will not be conducted during this period. If no payment is made within this grace period, the Membership will automatically terminate.
Refund Policy: No refunds will be provided. Unless otherwise provided by law, you acknowledge that all sales are final and we do not offer refunds for any portion of your payment for any of the Membership at any time. By purchasing a Membership, you agree that you will not issue a chargeback for any payment made as you are fully aware of this Refund Policy and voluntarily consenting to it.
Scheduling: Clients assume responsibility for scheduling their Membership sessions. You’ll schedule your sessions through your lead therapist, on the same day and at the same time each week. Other arrangements are subject to CereShift’s scheduling availability. Please come prepared to start your sessions on time.
Rescheduling & Cancellation: If you need to reschedule or cancel a session, you need to do so at least 24 hours in advance of your scheduled time by sending a text message to your lead therapist; otherwise, this will be considered a missed session. Arriving for a session more than 15 minutes late may also be considered a missed session.
Forfeiture of Sessions: Missed sessions, as described above, will be considered used sessions and will be redeemed as one of your four sessions per Membership billing cycle.
Membership sessions can only be used by one person; they are non-transferrable.
Clients are responsible for cancellation, via a text or email to their lead therapist, at least 48 hours prior to their next billing date. Processed charges are non-refundable.
