CereShift Packages Terms and Conditions

The purchase of one of CereShift’s Iasis Therapy Packages (hereinafter referred to as “Package”) signifies your agreement to our terms and conditions, as follows:

Iasis Therapy Package Description

This purchase entitles the bearer to the number of Iasis Therapy Sessions designated in the purchased Package. Sessions must be used within 365 days, or they will be forfeited. 

Eligibility

Individuals who have been accepted as CereShift clients are eligible to purchase Packages.

 Investment

You agree that you are financially willing and able to make this Package purchase by choice, and that by so doing, you are not incurring any economic hardship in any way.

Scheduling

Scheduling: Clients assume responsibility for scheduling their sessions through their lead therapist, subject to CereShift’s scheduling availability. Please come prepared to start your sessions on time.

Rescheduling & Cancellation: If you need to reschedule or cancel a session, you must do so at least 24 hours in advance of your scheduled time by sending a text message to your lead therapist; otherwise, this will be considered a missed session. Arriving for a session more than 15 minutes late may also be considered a missed session.

Forfeiture of Sessions:  Missed sessions, as described above, will be considered used Sessions.

Complimentary Sessions

Complimentary sessions are limited to two (2) individuals per family.

Transfer of Sessions

10 and 20 Session Packages:
Sessions not used within the initial therapy period (usually the first 3-6 months) can be
     ♦  banked for future use by the client before they expire.
     ♦  donated to a current or potential CereShift client in need.

40 Session Packages:
The sessions in this package can be shared among
     ♦  immediate family members (parents and children)
     ♦  family members residing in the same household

Package Expiration

10, 20 and 40 Session Packages:  Iasis Therapy Sessions expire 365 days from the date of Package purchase.

Refund Policy

Unless otherwise provided by law, you acknowledge that CereShift’s limited Packages refund policy is only applicable:
     ♦  to packages purchased on the day of the client’s complimentary session
     ♦  within the first 14 days after purchasing a package

Refunds under this limited policy will be based on the purchase price minus the following deductions:
     ♦  $200 per session used (after the complimentary session)
     ♦  a 5% administrative fee on the full package price

Refunds may take up to 14 days to process.

By purchasing a Package, you agree that you will not issue a chargeback for any payment made as you are fully aware of this Refund Policy and voluntarily consenting to it.
